As a Home Health Caregiver with BrightCare HomeCare, you will play a vital role in providing compassionate and personalized care to individuals in their homes. Our team is dedicated to delivering high-quality services that cater to the unique needs and goals of each client. As a Home Health Caregiver, you will be responsible for providing direct patient care, maintaining a clean and safe environment, and collaborating with our multidisciplinary team to ensure the best possible outcomes.

Responsibilities:

• Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting
• Maintain accurate and detailed records of patient care and progress
• Implement and update patient care plans in collaboration with the healthcare team
• Assist with medication administration, wound care, and other medical treatments as needed
• Maintain a clean and safe environment, including laundry, housekeeping, and meal preparation
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Collaborate with our multidisciplinary team to develop and implement individualized care plans
• Participate in continuous quality improvement initiatives to enhance patient care and outcomes

Requirements:

• High school diploma or equivalent required
• Certification in home health care or equivalent experience
• Ability to lift, transfer, and reposition patients, as needed
• Effective communication and interpersonal skills
• Ability to work independently and as part of a team

Work Environment:

• Work in clients' homes in the Gretna, Louisiana area
• Must be able to travel to and from client homes, as needed
• Ability to work varied shifts, including days, evenings, and weekends
