About the Role:
Join Vitalis Healthcare Services LLC as a Home Care Visit Nurse LPN, where you will play a vital role in delivering exceptional care to our patients in the comfort of their homes. Responsibilities:
- Conduct regular health assessments and monitor vital signs of patients in home settings.
- Administer medications and treatments as prescribed by physicians.
- Provide education and support to patients and their families regarding care plans.
- Document patient progress and report any changes to the healthcare team.
- Collaborate with interdisciplinary teams to optimize patient care.
- Ensure compliance with all safety and health regulations.
- Assist with personal care tasks, including bathing, dressing, and mobility support.
- Maintain accurate and timely records of patient care activities.
Requirements:
- Current LPN license in the state of Maryland.
- Minimum of 1 year of nursing experience, preferably in home care or similar settings.
- Strong assessment and clinical skills with attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor and a passion for patient-centered care.
- Basic Life Support (BLS) certification is preferred.
- Reliable transportation and willingness to travel to patient homes.
