How you'll make an impact in this role
  • Strategic Care Orchestration: Coordinate the comprehensive interdisciplinary plan of care from admission to discharge, integrating clinical pathways with the unique preferences, cultural values, and goals of the patient and their family.
  • Clinical Monitoring & Evaluation: Continuously monitor patient progress through direct assessments and medical record reviews to evaluate the effectiveness of the health plan and ensure the appropriateness of services relative to the diagnosis.
  • Resource Advocacy & Barrier Removal: Proactively identify and eliminate social, financial, or administrative barriers to recovery by advocating for necessary medical resources and facilitating access to post-discharge care options.
  • Interprofessional Collaboration & Documentation: Maintain active dialogue with healthcare providers and supervisors to ensure effective plan implementation while serving as a resource for high-quality medical record documentation and regulatory compliance.
What minimum requirements you'll need

Licensure / Certification / Registration:

  • Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date

    or job transfer date required. Licensed as a Registered Nurse in Florida required. Credentialed from State Board of

    Nursing or current home state license for multi-state license recognition "Compact State" obtained

    prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of

    hire.

Responsibilities
  • Coordinate the comprehensive interdisciplinary plan of care from admission to discharge, integrating clinical pathways with the unique preferences, cultural values, and goals of the patient and their family.
  • Continuously monitor patient progress through direct assessments and medical record reviews to evaluate the effectiveness of the health plan and ensure the appropriateness of services relative to the diagnosis.
