Benefits: Flexible schedule 401(k) About the Role: Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our dedicated home health team in Santa Clarita, CA. This is a fantastic opportunity for a registered nurse who values flexibility while making a meaningful difference in patients' lives right in their own homes. Responsibilities: Conduct skilled nursing assessments and provide direct patient care in a home health setting Develop, implement, and update individualized patient care plans Administer medications, wound care, IV therapy, and other clinical treatments as ordered Monitor and document patient progress, vital signs, and changes in condition Educate patients and family members on disease management, medications, and care techniques Collaborate with physicians, therapists, and interdisciplinary care team members Ensure compliance with Medicare, Medicaid, and home health regulatory standards Requirements: Active California Registered Nurse (RN) license in good standing Minimum 1 year of clinical nursing experience; home health experience preferred Current BLS/CPR certification required Valid California driver's license and reliable personal transportation Strong knowledge of OASIS documentation and Medicare home health guidelines Excellent communication, time management, and organizational skills Compassionate, self-motivated, and comfortable working independently in the field About Us: Karing Hearts Home Health is a trusted provider of quality in-home healthcare services in the Santa Clarita Valley and surrounding communities.
