Family Practice/Primary Care Nurse Practitioner

We are seeking a dedicated and skilled Nurse Practitioner to join our team at WCS Healthcare Partners. This role involves providing comprehensive care to residents across two skilled nursing communities, focusing on subacute, long-term care, assisted living (including memory care), and independent living environments. The ideal candidate will have a strong background in managing complex cases and exhibit excellent communication skills in a family-facing environment. Responsibilities Conduct morning rounds focusing on subacute residents and those with priority needs. Hold afternoon office hours dedicated to independent living patients. Collaborate with telehealth partners to ensure seamless after-hours coverage. Provide occasional coverage when the attending physician is on vacation, with compensation. Requirements Certified as an FNP, ANP, or AGNP. At least 1 year of experience in subacute or hospital settings dealing with complex cases. Ability to flex across different levels of care. Strong communication skills in a family-facing environment.
