Supervisory Social Worker (Patient Aligned Care Team)

Summary The incumbent works for the Fredericksburg Health Care System (FxHCC) in Fredericksburg - Virginia - as a direct report to the Patient Aligned Care Team (PACT) and Associated Programs Social Work Supervisor. Duties This position provides supervision to the PACT social work team and will further manage the program needs of the PACT social workers - to include collaborating with the interdisciplinary leadership for PACT at the Fredericksburg HCC. The incumbent will ensure that PACT social work services and documentation meet National and Local Social Work Guidelines - Adhere to Medical Center Policies (MCP) and Standard Operating Procedures (SOP). The incumbent will provide direct clinical social work services in the PACT department including biopsychosocial assessments, case management and crisis intervention. Performs other directly related duties as assigned.
