Job Description: Patient Health Advocate (PHA)

Position Summary

The Patient Health Advocate (PHA) plays a vital role in improving patient outcomes by combining data analysis, quality improvement, patient outreach, and care coordination. This position is responsible for identifying care gaps through data mining, scheduling patient appointments, auditing provider documentation, educating patients on preventive health measures, and supporting quality initiatives. The ideal candidate is organized, detail-oriented, and committed to delivering exceptional patient care while meeting organizational quality and performance goals.

Essential Duties and Responsibilities

• Perform data mining to identify patients who are due for preventive services, chronic disease management, and quality measures.
• Submit and update quality measure data accurately and in a timely manner.
• Contact patients by phone to schedule Annual Preventive Exams (APE), follow-up visits, and other required appointments.
• Audit provider charts to ensure documentation is complete, accurate, and compliant with quality measure requirements.
• Monitor and track patient care gaps and work collaboratively with providers and clinical staff to ensure timely completion.
• Provide patient health education regarding preventive screenings, chronic disease management, immunizations, and healthy lifestyle choices.
• Participate in departmental and provider meetings to discuss quality initiatives, patient outreach progress, and performance improvement strategies.
• Maintain and update reports using Microsoft Excel and Google Sheets to track productivity, patient outreach, and quality performance.
• Review medical documentation and verify appropriate CPT billing codes related to quality measures and preventive services.
• Collaborate with providers, medical assistants, referral coordinators, and administrative staff to improve patient care and operational efficiency.
• Maintain patient confidentiality and comply with all HIPAA regulations and organizational policies.
• Perform other duties as assigned to support clinic operations and quality improvement initiatives.
• Conversating with patients in Spanish.

Qualifications

Education

• High School Diploma or equivalent required.
• Certified Medical Assistant (CMA) certification required.

Experience

• Previous experience in a medical office, primary care, or quality improvement environment preferred.
• Experience with electronic health records (EHR/EMR) systems preferred.
• Experience with patient outreach and appointment scheduling preferred.

Knowledge, Skills, and Abilities

• Strong knowledge of medical terminology.
• Working knowledge of CPT billing codes and preventive care guidelines.
• Proficient in Microsoft Excel and Google Sheets.
• Excellent organizational and time management skills.
• Strong analytical skills with the ability to interpret healthcare data.
• Excellent verbal and written communication skills.
• Ability to educate patients in a compassionate and professional manner.
• Ability to work independently while collaborating effectively with a multidisciplinary healthcare team.
• Strong attention to detail and commitment to accuracy.
• Ability to prioritize multiple tasks in a fast-paced healthcare environment.
• Fluent Spanish required.

Physical Requirements

• Prolonged periods of sitting and working at a computer.
• Frequent telephone communication with patients.
• Occasionally standing, and walking

Work Environment

This position is performed in a medical office setting and requires frequent interaction with patients, providers, and clinical staff. The PHA is expected to maintain professionalism, confidentiality, and a patient-centered approach while supporting the organization's quality and population health initiatives.
