Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• VA Physical Therapist license required to start.
• Physical Therapist credential verification required to start.
• New graduates are not accepted.
• Hep B required to start.
• TB required to start.
• Flu required to start.
• Tdap required to start.
• Criminal Background required to start.
• OIG Search required to start.
• Statement of Good Health required to start.
• OSHA Packet required to start.
• Verification of Professional Education required to start.
• I-9 Form required to start.
• GSA Search required to start.
• UDS - 9 Panel (Marijuana Not Included) required to start.
• Contract Therapist Compliance Attestation required to start.

Assignment Details

• Shift Hours: 8 hours.
• Patients are scheduled every 30 minutes and treated for 60 minutes.
• Evaluations are scheduled for 60 minutes.
• Productivity expectation includes 11+ visits per day, 90% productivity, and 55+ visits per week.
• Documentation system: NextGen.
• Patient population includes adolescent, adult, geriatric, and orthopedic patients.
• Staff support includes Patient Experience Coordinators and Rehab Aides.
• Dress code: Business Casual.
• Temp-to-perm opportunity available.
