License: Current pharmacist licensure in Florida

Certifications: BLS and ACLS preferred but not required

Must-Have: PharmD or Bachelor's degree with recent acute care hospital experience

Advanced skills in verifying pediatric oral and intravenous solutions

Experience in pharmaceutical calculations and pharmacokinetics dosing

Ability to perform TPN calculations

Description: The clinical pharmacist practices pharmacy with the utmost code of ethics. They are responsible for the safe, effective, evidence-based, and cost-effective use of medications. The clinical pharmacist utilizes advanced knowledge, skills, and professional judgment in all work aspects. They serve as a patient advocate when planning and carrying out pharmaceutical care within a multidisciplinary team. Collaboration with other healthcare professionals is key. Adherence to all regulatory and medication safety guidelines is paramount. Responsibilities include verifying computerized physician order entry (CPOE) orders for accuracy and appropriateness. The pharmacist interprets medication orders, accurately transcribes them to computerized patient medication profiles, and documents as required. They prepare and dispense medications, including sterile and parenteral nutrition preparations, safely and accurately. Development and execution of drug therapy management protocols is based on their expertise. Issues related to controlled substances must be maintained following legal requirements. The pharmacist reads, extracts, and interprets information in patient charts accurately. Suspected adverse drug reactions and medication errors must be reported in a timely manner. Participation in the performance appraisal of pharmacy support personnel is expected. The work environment includes a focus on patient-centered care.
