Location: Chandler, OK
Job Summary:
We are seeking a motivated and compassionate Physical Therapist to join our team at Chandler Care Center. Responsibilities:
Conduct thorough evaluations and create personalized treatment plans to promote optimal physical function and well-being
Implement and modify treatment plans as needed to ensure effective rehabilitation outcomes
Collaborate with interdisciplinary teams to ensure comprehensive care and communication with residents, families, and other healthcare professionals
Provide education and guidance to residents and their families on proper body mechanics, safety, and wellness techniques
Develop and maintain accurate and complete records of patient treatment and progress
Participate in quality improvement initiatives to ensure the highest level of care and patient satisfaction
