Key Responsibilities\n \n Conduct comprehensive assessments of patients’ physical abilities and needs. \n Develop and implement individualized treatment plans to address patients' rehabilitation goals. \n Monitor and document patient progress, making adjustments to treatment plans as necessary. \n Collaborate with other healthcare professionals to ensure cohesive patient care. \n Educate patients and their families on treatment plans and exercises to promote recovery. \n Qualifications\n \n Valid New York Physical Therapist license is required. \n Basic Life Support (BLS) certification is required. \n Prior experience in a Skilled Nursing Facility is mandatory. \n Schedule\n This position requires availability on Saturdays from 8:00 AM to 4:30 PM.
