• Key Requirements Summary: 1. Experience: 1 year home health experience required; 1st time traveler acceptable 2. Cases: Geriatrics, Neurology, Orthopedics, Home Health, Post acute care, Post SNF discharge; OASIS experience required (total months and frequency of starts of care for Medicare patients per week must be included with submission) 3. Certifications: BLS (AHA) required; Wound Care and Manual Therapy listed 4. License: HI (Hawaii) License required 5. Education: Not specified 6. EMR: HomeCare HomeBase (HCHB) preferred; productivity requirement of 25 units per week 7. Shifts/Call: Floating to cover Oahu, Kauai, and/or Big Island may occasionally be required; all float expenses paid; 5 visits per day; orientation is 40 hours 8.
