Nurse Practitioner

Overview Physician Extender APP, Nephrology position at ChenMed in Berwyn, IL. The role involves coordination of quality care for CKD and ESRD patients between primary care physicians, community nephrologists, and dialysis clinics. ESSENTIAL JOB DUTIES/RESPONSIBILITIES Work directly with the National Director of Nephrology to analyze care gaps and share recommendations with PCPs and dialysis clinics to reduce patients crashing into dialysis. Coordinate care between ChenMed Nephrology team, PCPs, dialysis units, and vascular or inpatient facilities. Review patient charts and provide assessments and recommendations for care plans. Enhance home dialysis utilization, with an emphasis on increasing de‑novo starts of home dialysis. Collaborate with the transitional care team to decrease ESRD hospitalizations and length of stay. Provide virtual education to patients on CKD and dialysis options. Develop trusting relationships with internal and external stakeholders to identify opportunities and implement remediation strategies. Support and complete ad‑hoc initiatives, including research and care advancement proposals. Travel up to 75% as required. Perform other duties as assigned. Scope of Work and KPI's Late‑stage CKD (near dialysis) patients: Analyze gaps in care and recommend actions to Dr Kheda/PCP. Manage nephrology and transplant referrals. Review charts and assess candidacy for home dialysis. Assist in developing CKD patient education materials. Provide virtual education on CKD and dialysis options. Coordinate with community nephrologist to avoid crash‑in‑dialysis. Increase de‑novo start of home dialysis. Reduce crash‑in‑dialysis events. ESRD on hemodialysis: Analyze gaps in care and recommend actions to Dr Kheda/PCP/dialysis clinics. Conduct virtual assessment for potential transition to home dialysis. Coordinate with TCM team to reduce ESRD hospitalizations and LOS. Facilitate coordination among ChenMed, dialysis unit, vascular, and inpatient care. Increase proportion of patients on home dialysis. Reduce hospitalization rates and LOS. EDUCATION AND EXPERIENCE CRITERIA Licensed Physician Assistant or Nurse Practitioner with 5+ years of nephrology experience. Nephrology CAQ or Certified Nephrology Nurse (CNN) preferred. Experience with care coordination and coaching. Excellent verbal and written communication skills. Intermediate computer skills with Microsoft Office Suite, including Excel and Word. Medicare and/or Medicare Advantage experience a plus. Prior experience in value‑based care (e.g., ESCO) or risk‑model based healthcare a plus.
