Remote Value-Based Care Pharmacy Tech: Improve Adherence

Baptist Health is seeking a remote Value Based Care Pharmacy Technician to join our Pharmacy team. You will identify at-risk patients, conduct outreach to improve adherence, coordinate refill reminders, and assist with financial aid and insurance navigation. You will work under a licensed pharmacist to support transitions of care, document interventions in the EHR, and monitor quality metrics like Medicare Star Ratings and HEDIS, while collaborating with care teams to improve outcomes. #J-18808-Ljbffr
