Primary Care Physician— Venice, Florida

The center provides Uber/Lyft to those who don’t drive. Also, they offer exercise, art and other activities for their seniors to promote better health at no cost to the patient. \n

Role:

\n

\n
• Compensation: Base $260,000-$290,000 + up to 20% of base salary in bonus’
\n
• Bonus’ based on: 1. Patient satisfaction rating 2. Living the company’s values 3. HEDIS compliance 4. Reducing hospital sick days
\n
• Primary Care – Outpatient
\n
• Value Based Care Model
\n
• Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.)
