Clinical Social Worker II/III - Inpatient Psych

Job Description Under the general supervision of the Director of Clinical Services and as part of a multidisciplinary treatment team, the inpatient social worker’s duties include but are not limited to psychosocial assessment, case management, individual and family therapy and crisis intervention for patients hospitalized on an acute psychiatric unit. The social worker will provide timely and appropriate discharge planning taking into consideration the patient’s individual goals, preferences, needs and resources. Responsibilities Maintain a caseload of patients hospitalized on the acute psychiatric unit for adults and provide psychosocial assessment, case management, individual patient/family meetings, treatment planning and discharge planning. Provide psycho‑education to patients and families. Communicate effectively and maintain positive relationships with community partners (e.g., SFDPH, Conservatorship Office). Maintain up‑to‑date knowledge of community resources and referral processes to support discharge planning. Facilitate psycho‑education groups as needed. Serve as a clinical consultant to a multidisciplinary treatment team; facilitate referrals and services and participate in ongoing assessment of treatment services and training of medical students, residents and post‑graduate trainees. Provide individual psychotherapy, outreach, and case management as indicated by programmatic or patient care needs. Document patient care timely and in accordance with LPPH, TJC, Medicare and private funding regulations. Attend staff meetings and other duties as assigned, including maintaining knowledge of clinical service standards, infection control, patient safety regulations and participation in LEAN Management initiatives such as huddles and True North Board rounding. Participate in facilitation of Patient Advisory or Focus Groups on behalf of LPPH. Qualifications Master’s degree in social work, psychology, or related field from an accredited university. For CSW2, eligible to register as an associate with the Board of Behavioral Sciences in the state of California. For CSW3, Licensed with the Board of Behavioral Sciences in the state of California (LCSW, LMFT). Minimum 3 years relevant experience. Recent direct patient care experience in a mental health setting, group and crisis management skills. Demonstrated ability to provide transitional, comprehensive mental health services including psychotherapy, crisis intervention, outreach, and case management to a caseload of severely mentally ill adults who are high risk for psychiatric hospitalization. Demonstrated ability to complete psychosocial/functional assessments, mental status examinations, comprehensive treatment development, and discharge planning. Excellent documentation and clinical skills. Ability to prioritize; work independently, excellent follow‑up, work well under pressure and meet deadlines. Demonstrated excellent attendance and reliability. Demonstrated ability to assess suicide and violence risk. Demonstrated ability to work harmoniously with colleagues as a member of the treatment team. Excellent communication and interpersonal skills. A preliminary job offer and assignment may be extended pending successful background clearance. This position requires flexibility to orient and work at all UCSF Medical Center locations. Preferred Experience with populations diverse in ethnicity, gender, and sexual orientation. Experience with severely mentally ill. Licenses/Certifications For CSW2, eligible to register as an associate with the Board of Behavioral Sciences in the state of California. For CSW3, Licensed with the Board of Behavioral Sciences in the state of California (LCSW, LMFT). Salary Information The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF.
