Work Environment

The work setting for this role is our vibrant outpatient office, fostering a dynamic and patient-centered atmosphere. Operating Monday through Friday from 8:00 AM to 5:00 PM, our facility is designed to support a variety of orthopedic conditions, promoting healing through physical therapy interventions. Your contributions will be crucial in shaping the patient experience and maintaining the positive culture of our practice.

Key Responsibilities

• Comprehensively examine, evaluate, and develop tailored treatment plans for patients presenting with functional limitations and disabilities, ensuring adherence to state practice acts.

• Perform thorough evaluations that include a review of patient history and specific assessments to formulate actionable recommendations based on documented results.

• Demonstrate expertise in implementing treatment protocols that align with evaluation findings, the individual patient's health status, and evolving healthcare demands.

• Collaborate with a multidisciplinary team of healthcare professionals to modify patient care plans as needed and document significant changes in patient conditions effectively.

• Engage in appropriate discharge planning and follow-up care, ensuring timely and accurate documentation of all patient care transactions, including initial evaluations, progress notes, and discharge summaries.

• Stay compliant with all corporate and clinical training requirements, while adhering to local, state, and federal regulations governing physical therapy practice.

• Take part in regular facility meetings and contribute to initiatives aimed at improving patient access and quality of care, actively engaging in Continuous Quality Improvement (CQI) projects.

Patient Population / Client Focus

You will work primarily with adult patients suffering from orthopedic conditions, post-surgical recoveries, and sports-related injuries. This role involves assessing each patient as an individual and designing an effective rehabilitation strategy that encompasses their specific needs, preferences, and unique challenges.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.

• Current state licensure as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.

• A minimum of two years of experience in an outpatient orthopedic setting, demonstrating a strong foundation in clinical techniques and patient care.

• Valid current CPR Certification.

Preferred Experience

• Experience with evidence-based practices and patient-centered care approaches.

• Familiarity with electronic medical record (EMR) systems that facilitate clinical documentation and patient tracking.

• Participation in multidisciplinary teams contributing to collaborative care strategies.
