In-Office | Tucson, AZ, United States

10/05 start date, 40hrs, Day shift.

Responsibilities

• Provide skilled home nursing care to patients per care plan and physician orders.

• Assess patient needs, monitor vitals, and report changes to the care team promptly.

• Maintain accurate, timely documentation of visits, interventions, and outcomes.

• Uphold infection control and safety standards in all home care settings.

• Educate patients and family on care procedures, medications, and safety practices.

• Develop and adjust care plans in collaboration with supervisors and physicians.

• Demonstrate productivity and meet visit targets through efficient routing and scheduling.

• Demonstrate accountability for personal performance and professional growth.

• Participate in team meetings and contribute to quality improvement initiatives.

• Identify and report supply needs and equipment issues to ensure uninterrupted care.

• Support transition of patients between care settings to maintain continuity.

Experience/Qualifications

• Licensed Registered Nurse (RN) with current state nursing license.

• Proven experience in home health care or skilled nursing visits (X+ months).

• Ability to assess vitals, monitor changes, and document accurately in EMR systems.

• Reliable time management and routing efficiency to meet visit targets.

• Certified Home Health Nurse (CHHN) or similar home health certification preferred.

• Active CPR/First Aid certification and emergency preparedness skills.

• Strong documentation skills with attention to compliance and care plans.

• Excellent communication with patients, families, and care team; educational skills.

• Ability to develop and adjust care plans collaboratively; participate in QA initiatives.

• High accountability for performance and professional growth; self-motivated learner.
