Responsibilities Provide skilled physical therapy assistant services under supervision in SNF settings. Develop and implement patient care plans to achieve functional goals. Collaborate with interdisciplinary team to optimize patient outcomes. Document assessments, treatments, and outcomes accurately in EMR systems. Educate patients and families on exercises, safety, and self-management. Participate in ongoing professional development and skill advancement. Adhere to contract terms, on-site work requirements, and facility policies. Demonstrate accountability for personal performance and meet quarterly targets. Experience / Qualifications Licensed Physical Therapist Assistant in good standing; SNF setting preferred. Provide skilled PTA services under supervision in SNF facilities. Develop and implement patient care plans with measurable functional goals. Accurate documentation of assessments, treatments, and outcomes in EMR systems. Identify and report safety concerns and potential risks promptly. Monitor progress; adjust treatments based on objective outcomes and data.
