MDS Coordinator Responsibilities:
• Coordinate the development, implementation, and maintenance of Individualized Comprehensive Assessment Plans (ICAPs) and other assessment plans as required
• Conduct and document standardized assessments, including Minimum Data Set (MDS) assessments, on all residents
• Ensure accurate and timely completion of MDS assessments and updates
• Collaborate with healthcare team members to identify and address resident care issues
MDS Coordinator Requirements:
RN/LPN
Experience in a long-term or short-term care setting
Strong communication, organizational, and leadership skills
Ability to work effectively in a fast-paced environment with minimal supervision
Proficiency in Electronic Medical Records (EMRs) and MDS software
