We are looking for a CNA for Full Time Weekend Nights to join our Hospice House/In Patient Unit team!  The Hospice Aide will provide personal care services to the terminally ill hospice patient as needed, under the direction of the nurse. To provide assistance and understanding to the family and in time of bereavement. To work as a member of the Hospice team in providing Hospice care within the guidelines of the IDG care plan.

Essential Functions/Responsibilities:

1. Under the direction and ongoing nursing supervision, provides personal care and assistance to hospice patients in accordance with the established plan of care, including:

● Assisting with bathing, oral care, skin care, grooming, and hair care.

● Assisting with toileting and the use of a bedpan or bedside commode.

● Assisting with transfers, positioning, and ambulation as specified in the plan of care.

● Assisting with range-of-motion exercises as instructed by appropriate professional personnel.

● Performing incidental household services essential to maintaining the patient’s health, safety, and well-being in the home.

● Preparing meals, including special diets, as directed by the plan of care.

● Obtaining and recording temperature, pulse, respirations, and blood pressure when assigned and appropriately trained.

● Assisting with essential shopping when included in the plan of care and no other appropriate resource is available. 

2. Provides compassionate care and emotional support to patients and their families while maintaining dignity, comfort, and respect.

3. Demonstrates effective communication and interpersonal skills, including active listening, patience, encouragement, and sensitivity to the needs of patients and families.

4. Observes the patient’s condition and immediately reports changes in the patient’s physical or emotional status, environment, or family/caregiver situation to the supervising nurse.

5. Performs assigned duties in accordance with the patient’s individualized plan of care and promptly communicates any inability to complete an assigned task or meet an identified patient need.

6. Maintains required competencies and demonstrates the knowledge and skills necessary to provide safe and appropriate hospice aide services.

7. Participates in required supervisory visits and performance evaluations in accordance with agency policy and applicable regulatory requirements.

8. Participates in annual performance evaluations and competency assessments as required.

9. Completes required in-service education and continuing education, including a minimum of twelve (12) hours of in-service education annually or as otherwise required by applicable regulations.

10. Maintains current certification and/or other credentials required by the state and agency.

11. Adheres to Standard Precautions, infection prevention and control practices, and applicable OSHA requirements.

12. Maintains current knowledge of infection prevention and infectious disease protocols and applies established practices when providing patient care.

13. Maintains patient confidentiality and safeguards protected health information in accordance with HIPAA, agency policies, and applicable privacy requirements.

14. Adheres to all agency policies, procedures, safety standards, and applicable federal, state, and local requirements.

15. Performs other duties as assigned by the Administrator, Director of Clinical Services, and/or supervising nurse.
