Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Washington State Physical Therapy license or an active PT compact privilege to practice in Washington.
• Current BLS/CPR certification.
• Valid driver’s license, proof of vehicle insurance, and access to a dependable vehicle. Mileage between visits will be reimbursed.
• Ability to perform home health duties including patient evaluations, development of plan of care, progress notes, and discharge summaries in accordance with state and CMS documentation requirements.
• Ability to coordinate care with interdisciplinary teams, supervise PTA and home health aides where permitted by state regulation and agency policy, and provide patient/caregiver education.
• Schedule flexibility to support patient needs, including participation in weekend rotation and potential holiday availability as required by the agency.
• Minimum Education Requirement: Bachelor’s Degree.
• Must be able to complete required state and agency background and screening processes.

Preferred Qualifications

• Previous home health experience preferred.
• Experience with HomeCare HomeBase (HCHB) preferred.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Shift schedule: Monday–Friday, 08:00–16:00 (client indicates M–F 8:00–4:30 in job notes; primary shift listed as Days 0800–1600).
• Weekly hour expectation: approximately 36 hours per week.
• Coverage area: South King County, WA. Candidate will visit patients across the agency’s service area and may need to cover nearby agencies as needed.
• On-call/weekend requirement: agency requires schedule flexibility and participation in a weekend rotation and potential holiday coverage per agency needs.

Job Responsibilities

• Evaluate patient functional status and all body systems as required for CMS documentation.
• Develop and implement physical therapy plan of care in coordination with physicians and the interdisciplinary team.
• Provide skilled treatment to relieve pain, restore or maintain function, and document progress per agency policy.
• Consult with nurse supervisors and physicians regarding patient responses and condition changes.
• Complete evaluations, progress notes for each visit, and submit documentation per agency timelines. Prepare written discharge summaries or status reports at least every 60 days.
• Participate in staff meetings, in-service education, and quality initiatives as requested by agency leadership.
• Maintain licensure and clinical competency through continuing education.

Professional Requirements and Conduct

• Adhere to state laws governing physical therapy practice and agency standards of practice.
• Maintain professional dress in solid-color scrubs without prints, per agency policy.
• Perform duties that may include supervision of PTA and home health aides consistent with state regulation and agency policy.
