Part-Time Weekend Telephonic RN Care Manager (Remote)

Humana, Inc. (CenterWell Home Health) is seeking a Telephonic Nurse Care Manager to support patients transitioning into and out of post-acute services from a remote, home-based setting. The role focuses on coordinating care across home health, hospice, and palliative options and ensuring timely follow-up and patient advocacy. You will work 20 hours weekly, Saturdays and Sundays 10am–8:30pm EST, with weekday training evenings and occasional field/office travel.
