The RN provides care to these patients via the utilization of the nursing process and nursing trends. The RN maintains up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the healthcare team. Initial assessments upon patient admission are completed, and appropriate plans of care are developed in accordance with unit standards based on patient and family needs. The RN implements the plan of care, nursing interventions, and patient care procedures. The plan of care is modified based upon continuous evaluation. The RN takes responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents. Effective communication with others is essential to resolve issues in a direct and private manner when necessary. Constructive feedback is offered to assist in the professional development of peers, and collaboration with colleagues is encouraged to expand knowledge. The RN identifies personal learning needs and takes initiative to address them. All departmental competencies are fulfilled. Patient Bedside Shift Reports are received at the beginning of each shift, ensuring the inclusion of patient input. Validating a safe patient room environment occurs at the time of shift change, along with updating of the patient communication board in each patient room. Patient care tasks and responsibilities are delegated to assigned team members, with validation of task completion throughout the shift. Basic RN patient care activities, such as assessments, interpretation of vital signs, and completion of physician orders, are documented. Admission and discharge processes for patients are efficiently managed. Maintaining patient safety is the highest priority, with compliance to safety measures such as fall precautions and pressure ulcer prevention. A positive attitude is maintained to promote a healthy work environment and a healing patient experience. Patient call lights are answered promptly to ensure effective triage and completion of patient requests. Critical thinking is used to evaluate overall patient condition, identifying and acting on changes in patient status through nursing judgment. Effective communication within the healthcare team promotes interdisciplinary collaboration and patient safety. Time management is crucial to enable a quick admission and discharge process for patients, facilitating completion of necessary testing efficiently.
