The ideal candidate will possess a strong background in patient care, assisted living, and home health services, demonstrating a genuine commitment to improving clients' daily lives through attentive caregiving and assistance with activities of daily living. This role offers an opportunity to make a meaningful difference by delivering personalized care tailored to each individual's needs.Duties:Assist clients with activities of daily living, including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary.Monitor and record vital signs, observe patient conditions, and communicate changes promptly to healthcare professionals.Prepare nutritious meals in accordance with care plans and dietary restrictions, ensuring safety and hygiene standards are maintained.Administer medications accurately following prescribed schedules while adhering to HIPAA regulations for patient confidentiality.Support clients with memory care needs related to Alzheimer's disease or dementia by implementing specialized techniques and maintaining a safe environment.Perform light housekeeping tasks such as cleaning, laundry, and maintaining a tidy living space to promote comfort and safety.Provide emotional support and companionship, fostering a positive environment that encourages independence and well-being.Qualifications:Proven experience in home & community care, assisted living, nursing homes, or long-term care environments.Knowledge of patient observation, medication administration, vital signs monitoring, and emergency procedures such as CPR and first aid.Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, or dementia-related conditions including Alzheimer's and memory care.Ability to perform heavy lifting safely when required and operate assistive devices like Hoyer lifts or wheelchairs.Strong understanding of HIPAA regulations to ensure client privacy and confidentiality at all times.Excellent communication skills combined with compassionate interpersonal abilities to connect effectively with clients from diverse backgrounds.Relevant certifications such as CPR/First Aid are highly desirable; prior experience in hospice or home health care is advantageous.
