The Senior Community Care (SCC) program provides coordinated, comprehensive care to some of our highest-risk health plan members. As an Advanced Practice Clinician, you'll play a key role in delivering personalized care that improves quality of life and health outcomes. Key Responsibilities

• Complete comprehensive patient assessments, including:
  • Review of medical history and medications
  • Physical examinations and vital signs
  • Appropriate screenings and point-of-care testing
• Support members through short-term sub-acute rehabilitation stays
• Maintain thorough and accurate clinical documentation, including ICD-10 coding
• Partner closely with interdisciplinary care teams to coordinate treatment plans
• Lead discussions around advance care planning and serious illness care
• Educate and counsel patients, families, and caregivers

Required Qualifications

For Nurse Practitioners:

• Current ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

For Physician Assistants:

• Active NCCPA certification and applicable state licensure

Additional requirements:

• Unrestricted state license (or ability to obtain before start date)
• Valid driver's license with reliable transportation
• Active DEA license/prescriptive authority, or ability to obtain after hire in accordance with state requirements
• Ability to establish a collaborative practice agreement when required
• Ability to work in a variety of clinical settings and perform essential job functions

Preferred Qualifications

• One or more years of APC experience, preferably in long-term care
• Experience in geriatric care, senior living, home care, or long-term care environments
• Familiarity caring for patients with complex medical, behavioral, social, or functional needs
• Knowledge of chronic disease management and geriatric medicine
• Experience facilitating advanced illness and end-of-life conversations
• Comfort using electronic medical records and healthcare technology

Compensation: Hourly compensation for this position typically ranges from $53 to $79 per hour , based on factors such as location, experience, education, and other job-related considerations.
