Job Description Provides comprehensive nutritional assessments by evaluating BMI, nutritional status, dietary tolerance, and individual health needs while conducting screenings and developing age‑appropriate nutrition education and treatment goals. Collaborates closely with patients, families, Registered Nurses, physicians, and other medical staff to coordinate care, address specialized needs, and connect clients with appropriate resources. Maintains accurate and timely clinical documentation in the Electronic Health Record, participates in care planning and clinical reviews, and communicates effectively with healthcare teams and facility personnel. Minimum Requirements License to practice as a Dietician in the State of Louisiana Be over 18 years of age. Possess a valid driver’s license Prior experience preferred Core Competencies Knowledge of Patient Centered Medical Home Model (PCMH) and team based approach to care. Knowledge and ability monitor and measure programs’ compliance with all applicable policies, funding and billing regulations, and laws. Conducts compliance audits related to documentation requirements, clinical quality, and program licensing regulations and accurately reports findings. Assesses need for staff / clinical training and provides effective training sessions. Knowledge and ability to monitor all aspects of quality assurance / improvement and to implement plans to further improve quality. Knowledge and ability to assist in monitoring programs’ compliance to applicable accreditation standards and implementing corrective action plans as necessary. Job Duties / Skills Required Assesses BMI, evaluates nutritional status, and identifies nutritional needs. Evaluates patient’s tolerance for prescribed diet. Provides age-appropriate education and instruction. Coordinates care with other medical providers for medically fragile patients. Participate in setting treatment plan goals/objectives. Conducts healthcare screenings and assessments. Interviews clients, families, etc. and completes assessments and other clinical documentation. Communicate effectively with clinic staff, patients, and families. Enters information into an Electronic Health Record on all client services in a timely manner. Consult with facility personnel and administrative field personnel; collects and analyzes data to identify needs for the assigned special services program within the particular service area. Coordinate resources outside of Start when necessary to meet client’s specific needs. Maintain a clear driving record in order to be able to drive company vehicles when necessary. Attends all staff meetings as scheduled. May assist with clinical consultation, clinical review, or auditing of clinical records. Coordinates medical treatment with Registered Nurse and other medical staff.
