Role Responsibilities: Provide professional, compassionate nursing care in clients’ homes Perform assessments and manage individualized care plans Accurately document care using OASIS/Kinnser Manage diverse patient needs in a home health setting Respond to PRN and urgent visit needs as assigned Qualifications: Graduate of an accredited nursing program Active Texas RN License Valid Driver’s License and reliable transportation Minimum 1 year of home health experience Experience with OASIS/Kinnser preferred Ability to travel locally within the Fort Worth area Flexible availability for PRN visits Work Location: Client homes throughout the Denton area Field-based / on-the-road role Ultimate LLC , a Pillar Health Group Company, values your expertise and empowers you to make a difference—one visit at a time.
