Job Summary Woonsocket, RI Contract **Job Summary**: Nurse Practitioner leverages administrative support and delivers patient care services through virtual technology. The ratio of work is dependent on patient demand. You will collaborate with a team of professionals as you independently provide holistic, evidence‑based care, including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. Encounters are documented using an electronic health record (EHR). This role reports directly to the Minute Clinic Virtual Care SPM. Responsibilities Evaluate primary care, acute, chronically ill, and transitional care patients, providing education, counseling, and disposition planning for patients aged 18 months and above. Provide counseling on pregnancy prevention, STI prevention, contraceptive care, and medication management. Educate patients on health maintenance and respond to inquiries. Document all patient care in an EHR according to policies and procedures. Coordinate care with internal and external colleagues, including the broader patient‑centered medical home, to ensure the highest standard of care. Work within a patient‑care team including providers, physicians, pharmacists, and other health‑care members. Deliver excellent customer service, increase patient engagement, and create a welcoming environment. Tailor communication style to influence quality outcomes and patient needs. Collaborate with pharmacy and front‑store colleagues to provide a complete patient experience. Maintain confidentiality in line with PHI and HIPAA standards. Resolve conflict using management techniques. Maintain positive relationships among practice employees, health colleagues, and external partners. Reprioritize tasks to meet patient and business needs. Support clinical and non‑clinical duties to manage expectations and expectations. Adopt new service offerings and patient‑care models quickly. Assist with hiring, development, and evaluation of practice staff. Perform clinic operations: inventory, lab results, incoming calls, supplies, and cleanliness.
