Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment. To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care. Appropriately assesses patients of varying diagnoses and disorders requiring Physical Therapy according to established guidelines. Age range treated includes neonatal, pediatric, adolescent, adult, and geriatric. Staff will be assigned only age ranges for which they meet education, experience and training. Patients 0-21 years old with CCS coverage are to be treated by CCS paneled therapists or oversight by a CCS paneled therapist. Establishes functional goals in collaboration with the interdisciplinary team based on the patient's rehabilitation potential. Develops, implements & modifies treatment plan in collaboration with interdisciplinary team to achieve goals. Utilizes evidence based practice as available. Incorporates patient/family/Care Partner needs, interests/ goals in the development and execution of treatment plan. Provides patient/family/Care Partner/caregiver education, initiating as soon as feasible after admission. Works to ensure transfer of knowledge using a variety of techniques (i.e. teach back, demonstration, video, written handouts). Establishes and follows patient precautions. Observes patient's responses to treatment and reports unusual occurrences. Follows hand off guidelines.
