Outpatient Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patients' needs, goals, and physician directions. This role involves delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective returns to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves, while upholding professional standards and ethical conduct in accordance with the organization's mission and core values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Complete medical record documentation accurately and in a timely manner.
• Provide care under signed, valid physician orders and within insurance approval.
• Participate in the student program by supervising and developing learning objectives.
• Create an individualized professional development plan with clinical management.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure.
• Understand and follow organizational performance improvement methodologies.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an Accredited School.
• 6 months to 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable license for the state where work will be performed.

Schedule

This position requires a flexible schedule with 40 hours per week on a 5x8.0 shift.
