We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. Your Role in our Company Will Be The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks. The LVN is a licensed nurse who works in an Home Care Setting to provide nursing services.

AVAILABILITY:

Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer) Job Type: Per diem Pay:$45.00 per visit Medical Specialty: Home Health / Hospice Experience: Visiting LVN for Home Health: 1 year (Required) Nursing: 1 year (Required) License/Certification: Current CA LVN License (Required) BLS from the American Heart Association and/or the American Red Cross (Required) NSO Nursing License Insurance (Required) IV/Phlebotomy Certified (Preferred) Work Location: The city close to you, with a cluster of patients who need care for that day

RESPONSIBILITIES:

Providing nurse visits to home health patients. (home visits) Care for ill, injured, or convalescing patients by Implementing plan of care initiated by RN Provide basic patient care and treatment. Document consistently with the plan of care. Communication with patients to schedule visits. Educating the patient how to take care during non-visit days Demonstrate knowledge of regulatory requirements.
