What You'll Do As an LPN, you will play an important role in providing safe, compassionate, and individualized care to our residents. Responsibilities include: Assessing resident needs and monitoring changes in condition Developing and implementing individualized care plans Providing direct nursing care and ensuring residents' needs are met Supervising CNAs and other care team members in the delivery of nursing services Maintaining accurate and timely clinical documentation Following applicable state and federal regulations, facility policies, and nursing standards Communicating effectively with residents, families, providers, and the interdisciplinary care team Advocating for residents and helping promote their comfort, dignity, and quality of life What We're Looking For Current, active Licensed Practical Nurse (LPN) license in the state of Oregon Ability to assess resident needs and respond appropriately to changes in condition Knowledge of individualized care planning and nursing practices Ability to supervise and work collaboratively with CNAs and other healthcare team members Strong communication and organizational skills Compassion, dependability, and a genuine desire to make a difference in the lives of others Commitment to following state and federal regulations governing skilled nursing care Newly licensed LPNs are welcome to apply!
