Are you a compassionate Licensed Practical Nurse who enjoys building meaningful relationships with clients while providing one-on-one care? We are seeking dedicated LPNs to join our growing home care team serving Mankato and the surrounding area.
As a SYNERGY HomeCare LPN, you'll provide services in clients' homes, helping them maintain their independence while improving their quality of life.
Responsibilities
- Providing direct care according to individualized care plans
- Monitor client health status
- Medication reminders and treatments as directed by PT/OT.
- Accurately document all visits and nursing interventions.
- Communicate effectively with the SYNERGY HomeCare Admin Team and SYNERGY Registered Nurse.
- Ensure compliance with state and federal home care regulations.
Qualifications
- Current Minnesota Licensed Practical Nurse (LPN) license in good standing.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel throughout Mankato and surrounding communities.
- Excellent communication and organizational skills.
- Home care experience preferred but not required—we're willing to train the right candidate!
