Outpatient Clinical Dietitian

LifeBridge Health is seeking an Outpatient Clinical Dietitian to join our Healthcare & Medical Services team in delivering compassionate, evidence-based nutrition care. In this role, you will provide individualized medical nutrition therapy, conduct comprehensive assessments, and develop patient-centered care plans for diverse outpatient populations, including those with chronic diseases. You will collaborate closely with an interdisciplinary care team, document in the EHR, and contribute to quality improvement and community wellness initiatives. Responsibilities Provide individualized outpatient medical nutrition therapy for diverse patient populations Conduct comprehensive nutrition assessments, develop care plans, and monitor outcomes Collaborate with physicians, nurses, and allied health professionals to coordinate care Educate patients and families on chronic disease management, lifestyle change, and wellness Document care accurately in the electronic health record and maintain regulatory compliance Participate in quality improvement, community outreach, and program development initiatives Precept or mentor students and support ongoing professional education activities Core Competencies Medical nutrition therapy Outpatient nutrition counseling Nutrition assessment (adult and pediatric as applicable) Electronic health record (EHR) documentation Care planning and goal setting Motivational interviewing Interdisciplinary team collaboration Chronic disease management (diabetes, renal, cardiac) Nutrition education materials development Quality improvement and outcomes tracking Benefits Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Requirements Registered Dietitian Nutritionist (RDN) credential required or eligibility within 6 months State licensure as a dietitian/nutritionist in Maryland (or ability to obtain) CPR/BLS certification preferred Experience with EHR systems preferred Certified Diabetes Care and Education Specialist (CDCES) preferred #J-18808-Ljbffr
