At Elara Caring, our Physical Therapists provide high-quality, compassionate care in the home setting. Youll promote the companys philosophy and mission while delivering patient-centered services.
- Provide direct patient care, evaluate functional needs, and consult with interdisciplinary team members.
- Develop and implement individualized care plans to optimize patient outcomes.
- Conduct therapy practice within defined standards and promote physical therapy within the community.
- Document evaluations, treatment goals, and progress accurately and timely.
- Direct and provide leadership to support staff in achieving patients functional goals.
Requirements:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license in the state of practice.
- Minimum of one year of experience in a healthcare facility.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to travel up to 50% within service area.
- Physical ability to frequently lift, push, and pull up to 50 pounds.
