Countryside Manor – Fremont, OH
Weekend Warrior Program | LPN
$8,000 Sign-On Bonus for Full-Time LPNs

Position Summary:
Countryside Manor is seeking dependable and compassionate Licensed Practical Nurses (LPNs) to join our Weekend Warrior nursing team . This opportunity is ideal for LPNs looking for a consistent weekend schedule while providing high-quality care in a skilled nursing and long-term care setting.

Join a supportive clinical team where your skills, dedication, and commitment to our residents are valued.

Key Responsibilities:

  • Provide direct resident care including medication administration, treatments, and nursing services

  • Monitor resident conditions and promptly report changes

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Complete accurate and timely clinical documentation

  • Collaborate with the interdisciplinary team to support resident care plans

  • Promote a safe, compassionate, and resident-centered environment

Weekend Warrior Schedule:

  • Weekend Warrior nursing position

  • Consistent weekend scheduling

  • Must meet Weekend Warrior program scheduling requirements

Compensation & Benefits:

  • $8,000 Sign-On Bonus for Full-Time LPNs (paid within first year)

Qualifications:

  • Active, unencumbered Ohio Licensed Practical Nurse (LPN) license

  • Current CPR certification
