Position Overview

We are seeking a dedicated Case Manager to join our healthcare team in Eureka, CA. This role requires acute care experience and is designed for candidates who have a passion for providing quality patient care and support.

Shift Information

• Shift Type: Day (5x8 Hour Schedule, 08:00 - 16:30)
• Weekend Requirements: Required to work every other weekend
• Call Duty: Call is not required
• Floating: Candidate will float between like units based on departmental needs

Qualifications

• Experience: Minimum of 2 years of acute care experience required
• First-timers: Candidates accepted with clinical approval
• Charting System: Experience with Epic is required
• Licensure: Valid Compact license must be in hand at submission

Core Responsibilities

• Conduct comprehensive assessments and develop patient care plans
• Coordinate care across the healthcare continuum
• Ensure compliance with CMS guidelines and HIPAA regulations
• Manage discharge planning and facilitate necessary support services
• Assist patients with admissions, benefits eligibility, and prior authorizations
• Collaborate with interdisciplinary teams to optimize patient outcomes

Preferred Skills

• Experience with HMOs and IPAs
• Familiarity with advance directives and protective services processes
• Knowledge of DRGs (Diagnosis Related Groups) and Core Measures
• Ability to manage complex cases including pediatrics and geriatrics
