Job Title: Registered Nurse Profession: RN Specialty: Rehabilitation Duration: 14 weeks Shift: Night Hours per Shift: 12.5 Experience: Minimum 2 years RN experience required. License: NYS RN License required. Certifications: BLS required. Must-Have: Excellent organizational skills. Description: Delivers patient care by incorporating the tenets of professional practice with a focus on relationship-centered care. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and discharge planning. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary care team. Manages assignments within the care delivery model effectively. Role models the principles of relationship-centered care through various patient engagement actions. Considers patient values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patient perceptions of care. Initiates service recovery when patients and family members have concerns. Demonstrates caring, respect, compassion, empathy, and active listening in interactions. Supports staff in using evidence-based practices to improve patient care perceptions. Conducts purposeful hourly rounding for assigned patients addressing key care aspects. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on evidence and safety procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to implement quality improvement projects and nursing research studies. Contributes to quality and safety practices. Implements principles to identify and avoid high-risk behaviors. Escalates potential safety hazards or gaps in best practices appropriately. Utilizes information technology by documenting nursing practice to support quality and performance improvement initiatives. Coordinates with nursing leadership to ensure supplies are appropriately utilized and available for patient care. Demonstrates patient-focused and cost-effective approaches to care in terms of resources. Documents nursing practice accurately and thoroughly. Facilitates patient throughput on a shift basis. Contributes to a supportive environment for clinical colleagues, patients, and families. Advances clinical competence in nursing practice. Projects a professional image and communicates with respect and empathy. Incorporates ethical principles into decision-making processes. Encourages scholarship, inquiry, lifelong learning, and innovation within the team. Acquires knowledge and skills relevant to the role and patient population. Participates in shared decision-making through practice councils and nursing department initiatives.
