On-Site Community Health Navigator (Batavia, NY)

Forward Leading IPA

Forward Leading IPA is seeking a Community Health Worker to navigate patients through healthcare resources in Batavia, NY. This role emphasizes building trust with patients, coordinating essential services, and engaging with Medicaid members. You will work closely with community partners and healthcare providers to improve access to care.

The ideal candidate has experience in human services and excellent communication skills. This in-person position is pivotal for supporting underserved populations through proactive outreach and integrated care efforts.
