Home Health Licensed Practical Nurse

Provide skilled nursing care to patients in their homes under the direction of an RN Implement individualized care plans with the patient, family, and healthcare team Educate patients and families on disease management, treatment options, and self-care techniques Maintain accurate records documenting patient visits, interventions, and outcomes Communicate promptly with patients, physicians, clinical managers, and healthcare team members regarding needs and status changes Participate in care coordination and discharge planning Support patients in regaining independence while managing chronic conditions or recovering from illness, injury, surgery, or hospitalization Requirements Current nursing license in the practicing state Valid driver's license Auto insurance Reliable transportation Current CPR certification Two years of experience as an LPN/LVN in a clinical setting Must complete TB screening if selected Must maintain personal vehicle liability insurance in accordance with state minimums or specified higher limits Home Health or Hospice nursing experience preferred Core Competencies Demonstrates expertise in providing skilled nursing care in home settings, implementing individualized care plans, and educating patients and families on disease management.
