Registered Nurse – Home Health

Schedule: 8 hours per shift

Assignment Details:

Contract Length: 14 weeks
Guaranteed Hours: 40 hours per week

Requirements:

Current state license as a Registered Nurse
Homecare Homebase Software Experience
OASIS Experience
1-2 years experience as a Registered Nurse in a clinical care setting
2+ years Home health experience
Proof of current CPR certification
Current driver's license with reliable transportation and insurance

Roles & Responsibilities:

Assess patients in home health or hospice settings to determine their needs and develop a comprehensive Plan of Care.
Coordinate with the interdisciplinary team, patients, and families to ensure continuity of care.
Document patient visits, evaluations, and changes in care promptly and accurately.
Educate patients and families on care plans, medications, and home safety, promoting self-care practices.
Supervise and provide clinical direction to auxiliary nursing staff.
Arrange referrals to other agencies when necessary.
