Job Summary:

As a Hospital Liaison for Focus Health Network, LLC in Pittsburgh, PA, you will play a critical role in ensuring seamless communication and collaboration between our healthcare teams and hospital partners. With a focus on delivering exceptional patient care, you will serve as the primary point of contact for hospital staff, providing timely and effective solutions to meet their needs. Responsibilities:

• Establish and maintain strong relationships with hospital staff, including physicians, nurses, and administrators

• Provide timely and effective communication to hospital partners, ensuring efficient and accurate exchange of information

• Coordinate and facilitate hospital-to-hospital and hospital-to-home transitions, ensuring continuity of care for patients

• Identify and resolve issues promptly, utilizing problem-solving skills and critical thinking to find creative solutions

• Collaborate with healthcare teams to develop and implement processes and procedures that promote efficient and effective care transitions

• Participate in quality improvement initiatives, analyzing data and implementing changes to improve patient outcomes and satisfaction

• Maintain accurate and up-to-date records, ensuring compliance with regulatory requirements and organizational standards

Requirements:

• 2 years of experience in healthcare administration, patient navigation, or a related field

• Strong communication, problem-solving, and organizational skills

• Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines

• Strong attention to detail, with a focus on accuracy and quality

• Ability to maintain confidentiality and handle sensitive information with discretion

• Strong knowledge of the healthcare industry, including regulations and best practices
