Solaris HealthCare, a Florida-based leader in skilled nursing and rehabilitation, is seeking a compassionate PTA to join our resident-centered team. In this role, you will provide direct physical therapy treatments under the direction of a Physical Therapist, focusing on restoring function, improving mobility, and promoting safety for senior residents. You will assist with evaluations, implement individualized care plans, document progress, and collaborate closely with nursing and rehab staff. Responsibilities Deliver physical therapy treatments under supervision of a Physical Therapist Assist with implementing and progressing individualized care plans for residents Provide gait training, transfer assistance, and therapeutic exercises Monitor, document, and report resident progress and response to treatment Educate residents and families on mobility, safety, and home exercise programs Collaborate with nursing, rehab, and interdisciplinary team members on care coordination Ensure compliance with facility, state, and federal regulations and documentation standards Maintain safe treatment areas and follow infection control and fall prevention protocols Participate in care conferences and contribute to discharge planning Support a resident-centered, compassionate care environment aligned with Solaris Health Care values Required Skills Physical therapy treatment implementation Gait and balance training Therapeutic exercise techniques Patient transfer and mobility assistance Care planning and documentation (EMR) Knowledge of geriatric conditions Infection control practices Interdisciplinary team collaboration Patient and family education Regulatory and compliance awareness (SNF) #J-18808-Ljbffr
