Description

• Conduct evaluations for patients in both inpatient and outpatient environments, crafting tailored treatment strategies
• Deliver targeted physical therapy interventions that enhance mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to ensure optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and exercises to be continued at home
• Meticulously document assessments, treatments, and patient progress in alignment with facility policies and regulatory standards
• Engage actively in care team rounds and interdisciplinary conferences to support comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent credential from a recognized program

Certifications

• Possess a valid state license as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Prior experience is preferred, with an emphasis on treating patients with intricate medical and mobility challenges
