Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum 1 year of case management experience required. License: CA RN License Certifications: BLS Must-Have: Home health case manager experience Description: An experienced RN is responsible for the overall management of patients. This includes the provision of direct care with a designated visit productivity standard. The position plans, organizes, and directs all patient care services for patients within the assigned caseload. Care is conducted in accordance with current standards and regulations, ensuring optimal quality care is maintained. The RN is responsible for coordinating the interdisciplinary team to implement the established plan of patient care. Effective management of visit utilization and control of expenses is essential. The RN may assist in agency-wide orientation, competency evaluations, and provide consultation to clinical staff related to specific expertise.
