In this role, you will provide direct patient care, focusing on promoting wellness, managing chronic conditions, and coordinating care with other healthcare providers. This position offers a rewarding opportunity to improve the health and quality of life for seniors within SNF, assisted and independent living communities.Essential Duties & ResponsibilitiesPerform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents, focusing on preventive care and chronic disease management.Conduct regular rounds to assess residents' conditions, adjust treatment plans, and address any emergent health concerns.Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the residents' needs.Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses to support better health outcomes.Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols.Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction within the facility.What We OfferWe know that caring for seniors takes more than just medical expertise—it takes support, tools, and opportunities to grow. Depending on the amount of travel required, Curana may in future determine that a valid driver's license, current auto insurance, and an acceptable driving record are required.
