Position Overview

The Family Medicine Physician provides comprehensive primary care services in an outpatient setting while supporting our commitment to whole-person, integrated healthcare.

This role partners with clinical and operational leaders to deliver high-quality, patient-centered care, improve population health outcomes, and advance Jane Pauley‘s mission of creating healthier communities.

What You‘ll Do Clinical Care

• Provide comprehensive primary care services to patients across the lifespan
• Deliver evidence-based, culturally responsive care that promotes positive health outcomes
• Maintain an assigned clinic schedule and provide on-site coverage as needed
• Participate in equitable call coverage with fellow providers
• Coordinate referrals and specialty care while supporting continuity of care

Team-Based Collaboration

• Work closely with nurses, medical assistants, behavioral health providers, care coordinators, and other interdisciplinary team members
• Support integrated care delivery and coordinated treatment planning
• Serve as a clinical resource and mentor to members of the care team
• Contribute to a positive culture focused on patient experience and clinical excellence

Quality & Population Health

• Participate in quality improvement and patient safety initiatives
• Support population health strategies and care gap closure efforts
• Engage in value-based care programs and performance improvement activities
• Contribute to the development of clinical pathways, best practices, and evidence-based care standards

Organizational Leadership

• Maintain compliance with all regulatory, accreditation, and licensing requirements
• Complete timely and accurate clinical documentation
• Participate in medical staff meetings, committees, and organizational initiatives
• Support recruitment, onboarding, and mentorship efforts as needed

Qualifications Required

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Board Certified or Board Eligible in Family Medicine
• Eligible for medical licensure in the State of Indiana
• DEA certification or eligibility
• Strong communication and interpersonal skills
• Commitment to serving diverse and medically underserved populations

Preferred

• Previous experience in an FQHC, community health center, or underserved setting
• Experience with value-based care and population health initiatives
• Experience working within a multidisciplinary care team
