Seeking an experienced Occupational Therapist (OT) for a 13-week home health assignment providing skilled occupational therapy services to patients in their homes. The Occupational Therapist will evaluate patient needs, develop individualized treatment plans, and implement evidence-based interventions to improve independence with activities of daily living (ADLs). This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating with the interdisciplinary healthcare team.

Requirements:

• 1 year of Home Health Occupational Therapy experience required
• Active Illinois Occupational Therapist license required
• BLS (AHA) certification required
• Valid driver's license and current car insurance required
• COVID-19 vaccination required (declinations will not be approved)
• OASIS and Start of Care experience strongly preferred
• Experience with electronic medical record documentation required
• Ability to work independently and manage a home health caseload required
• Two supervisory references required at submission
• Active license and certifications required at submission

Unit Details:

Responsibilities include evaluating patients, developing and implementing individualized treatment plans, providing therapeutic interventions to improve ADLs and functional independence, completing OASIS assessments when applicable, performing Start of Care visits, collaborating with physicians and the interdisciplinary care team, supervising OTAs and therapy students, educating patients and caregivers, documenting all patient care in the electronic medical record, ensuring compliance with AOTA standards and evidence-based practice, maintaining patient confidentiality, and providing safe, high-quality occupational therapy services in the home health setting.
