Registered Nurse (RN) Care Manager - $5,000 Sign-On Bonus | Hybrid Role | Eastern MA

We are seeking an enthusiastic RN Care Manager to join a mission-driven care management team based in Everett, MA . This hybrid role offers the perfect blend of in-home visits and remote coaching- no night shifts, weekends, or on-call duties -giving you the work-life balance you deserve. Key Responsibilities:- Conduct in-home visits to assess safety, monitor health conditions, and provide clinical coaching, including medication reviews.
- Follow structured care plans and offer ongoing support via a digital platform, including referrals and health coaching.
- Build supportive, trusting relationships with family caregivers to drive positive health outcomes and prevent burnout.
- Maintain timely and accurate documentation in compliance with regulatory standards.
- Participate in special projects and team initiatives as needed.
- Active Registered Nurse (RN) license in Massachusetts.
- Minimum of 2 years' recent experience in eldercare or supporting adults with disabilities.
- Valid driver's license and auto insurance; ability to travel for in-home visits.
- Strong background in case management, care planning, and assessments in complex care settings.
- Proficiency with MS Office and case management software.
- Bilingual candidates are encouraged to apply-language skills may positively impact compensation.
